2026 swim team 6 and under evaluation form

This form is for NEW 6 and under swimmers to Seven Hills only. If your child competed with the Seven Hills swim team in 2025 in the 6 and under category, you do not need to fill out this form. Parents - if you have more than one child requesting to join the 6 and under group, please fill this form out one time PER CHILD.

Upon filling out the form, one of our swim coaches will be in touch with you to schedule a swim team evaluation for your child (ren).
